Nicepage contact form still not working into Wordpress

I`ve tried to test Nicepage forms to work with Wordpress using a well hosted and .com domain name but it doesn´t work as before in other maybe "not well hosted domain name"... Configured Easy WP SMTP...

Text loses its cell when block duplicated
