Hi,

What exact labels do you mean? Do you mean that you want to add some labels in the subject of the email depending on some options chosen in the form? This is not possible, the subject is the same for all submissions from a single form.

I have customized a form to be a membership application. The labels are questions to be answered by the applicant. When the client receives the answers to the questions, the labels are not included, so they will know what question was responded to.

Here's a snippet of the form.

I figured it out - I had to change the names to match the labels. See attachment
