So far Nicepage help has been good. But I mention in my question the help pages don't help very much here, and you come with text from one of the help pages.
In your image under 2, do I put my email address there? Or something with the server? And under from, what do I put there, something from the server? The subject, is that the subject that I receive or something else?

In the E-mail Address you need to write the e-mail where all the form submits will be sent, where you want to receive them. The From option is for the e-mail that will be mentioned as the sender e-mail for the form submits. And the Subject is the subject of the e-mails that you will receive when the form is submitted.
Also in the article https://nicepage.com/doc/send-emails-with-php-script-62334 we have mentioned what is needed to be done on the server. That depends on the hosting you are using.

So I have 2 nicepage sites, one English, one Dutch, merging on the server into one. On 5 of the pages, so 10 in total, I have a contact from. Html site.
Contact form is in test working on some of them, exactly same form except for some information on input 'je email' in stead of 'your email' and on sending so I know where it comes from ' you have a new form from welkom' in stead of 'you have a new form from welcome'. But only on some of the pages sending is working. I copy from the working pages, upload to test again, and now some of the other ones are working. Now I don't change but upload again, and again some other ones are working.

How to solve this please, are the scripts generated the issue?

OK, got it. The problem was caused by incorrect copying of the scripts. All working now in test.

Could you possibly help and shed some light - I've designed 3 or 4 sites now using NicePage and all my forms work fine. Did a form now on a new site but the form just doesn't want to work? Using PHP script form.

Any advice or something to try?

If you are using the PHP form on a simple HTML site, most probably the issue is on the server side and some settings for the e-mail sending are not correct. Please check our recommendations and contact your hosting provider:
