The telephone dropdown field in the form displays incorrectly (shows a large scrolling text box displaying country telephone codes not uniformly set). How do I correct this problem please? Screen shot attached.

The screenshot I sent you is not from a phone, it's from the chrome browser. The countries are not visible, just flags and irregularly spaced telephone codes. The preview mode was not in phone view, it was in desktop view using chrome. Please advise.

Yes, I see that this is a desktop browser in your screenshot. That is why I asked you to check the issue on a real phone as mobile phones and the phone browser mode can display pages differently.
