Creating a form

Hi i am having a lot of trouble for creating a form with: Name, Phone, Email, Subject and message.
I have tried by URL and by PHP script but nothing works. The email need validation with username, password, port number, and host smtp. I've tried using PHP Mailer to help but without success.
The file i made using nicepage is "form-0310". I've changed the email for privacy reasons and changed to txt to send here.
The file i made using PHP mailer is form-1add---Copia.txt
How i do it?

Clarify please, does sending emails not work? Do you have any errors?

Please make sure that your server is configured for sending emails from Contact Forms. We recommend you to contact your hosting provider for getting more information concerning this subject.

Also, make sure that you have followed our instructions (1, 2) properly.
