Прошу подскахать как настраивать форму отправки, в смысле куда вставлять контактный Email&

I am creating a page with a contact form included. I am using PHP script. After pressing submit button no mail is sent but I am going to nicepage.com!? What's going wrong?

Can anyone advice how to make the form instead of one column become side by side with one submit button ?
