My old NetObjects 2015, I used in the past to do this kind of thing, has died so I can't give you a specific example of what I created myself. However, if you create a form with text headings and fields to fill in, I want this same form with the data the user types in the fields to be emailed in as close as possible the same look as the original form. In the ultra-simple example below where I was not able to include text colors or backgrounds, shapes, etc. that are possible with your Form Editor, I show item names such as 'Name' with the data entered in the field immediately following the item name. I expect all this to be emailed to me in this format ... everything that is in the original form.

- this is a space which should be output in the email
Name: _________________________ --data goes here, Name and data to be output in the email
Address: ______________________ --data goes here, Address and data to be output in the email

A form that I have created with Nicepage can be viewed at https://squaredanceman.com/Scholorship.html and my expectation is to see this same form in my email client window or sent as a pdf.
