How do I connect my website buttons to my forms?

I have several buttons on my website and now need to create forms with questions and link them to my buttons or create other completed galleries for additional photos to review.

What I do is create a "forms" folder on my host and upload the forms into the folder using Filezilla.
I then link to those forms using full url (https://www.site.com/forms/file.pdf)
