I have been following the instructions on how to pass subscriber information from a Nicepage form over to MailChimp by following these directions:

I followed the example and have verified that I can pass on subscriber information from text inputs easily (like FNAME, LNAME, and EMAIL).
However, I am struggling with passing information on from checkboxes into MailChimp. I want users to be able to select a Nicepage checkbox and have that map over to a MailChimp checkbox/group. I have followed a similar strategy in making sure the MailChimp name & value for the checkbox matches the Nicepage Name & Value attirbutes, but it doesn't seem to work.
Below is the page I am trying to use to solicit subscriptions. Right now I've only built in 1 checkbox, but ideally will expand that to 8 options if I can get it working with MailChimp correctly.
