Does anyone have any idiot guide steps that I could follow, or a template that is available?

Good morning all, I would like to create a database on my brand new Nicepage created site but I really don't know where to start. On my CPanel, I have created the database but I have no idea on how to create the form and post inputs to the database. Does anyone have any idiot guide steps that I could follow, or a template that is available? Thanks George.
