Please open the Nicepage editor, find the troublesome Contact Form, click on the first field and uncheck Autofocus. Save changes and republish your website.

Concerning the Back to Top button, this is the default behavior when it appears on scroll. Unfortunately, it cannot be changed.

Concerning the third question, do you want the name of a page to appear together with the received data from the Contact Form? If yes, we have added this feature to our wishlist.
