The email address that you input as Where to send submissions is actually receiving the message from the Contact form, is it correct?
The recipient does not receive a message. the one you have input in your Contact form on site for testing, is it correct?

Glad to know that the form is working ok now.
You can easily change the Label name for each form field

And add an additional field with mentioned options and named it as you need, please have a look:

Please adjust the Label and Name options of the form fields. Please use one word or "two-words" for the Name attribute.
