From the nicepage program you can't add that form in the hamburger menu, I added it manually after export.
I don't understand the logic that when I click ONLY on the input, the menu disappears ... and by the way, you could at least watch the movie to see what it's about, if I don't answer this to a robot ...disappointed

If I simulate from the desktop it works well but if I run from android the menu disappears (I tried from three different phones and three different browsers, Mozilla, Chrome, Brave, the same thing happens).

We've tried to test your website on other android mobiles, and it was responding the same as on your video.

After the inspection of the element and checking your website, we checked your code that you added.

Form which you have created is looking for the file "cpanel.php" and it's absent in the parent folder.

As we understood you want to create a registration form for your users.
Is that correct?!

It's not a registration, it's a login, but I don't see why that file would be a non-existent problem as long as I don't even get to click the button, that strikes me and I don't understand why it disappears, it's a simple form.
From my point of view it is a challenge both for you and for those who want to help ;)
