I notice that the field names used in the admin email differ depending on what sort of submit method is used, and which type of field used.
If multi use is selected, the email template uses capitalised element field names
If php is selected, then the element labels are used for text and textarea fields, but the element field name is used for the radio button control output.
I have not tried all controls, but those certainly need attention.
